Full payment is due, and expected, at the time services are rendered.

You will be responsible for paying any copays, deductibles, coinsurance, and charges for non-covered services at the time of service.

Please be aware that services provided may not be covered under your particular plan. Should that be the case, you will need to contact your insurance company with any questions concerning benefits and/or exclusions under your policy.

Any payment arrangements MUST be made with our billing department prior to your visit. To ensure proper payment, our office will verify insurance coverage and benefits using the information provided to us.
